Heart Failure Regional Program at SPH and VGH

Referrals for new diagnosis heart failure or suspected heart failure (with or without low ejection fraction) will be centrally triaged and assigned to the most appropriate site. The multidisciplinary care at sites includes patient education and optimization of therapy over multiple visits. On achieving optimal therapy, usually 6 months, patients are returned to the referring GP/specialist for ongoing care.

Appropriate referral will have at least 1 of the following indications:

• Positive BNP/NT-proBN{ (less than 3 months),
• a positive Echo,
• a Consult Note, or a Discharge summary (of less than 1 yr)
